Birth certificates in N.C.
Certified full copies of N.C. birth certificates may be obtained by writing or going to the N.C. Vital Records, 1903 Mail Service Center, Raleigh, NC 27699-1903, 919-733-3000, or going to http://vitalrecords.nc.gov.
Requests must include the child’s full name, date of birth, the mother’s full maiden name, father’s name, the city and county of birth, and the signature of both parents if the person is younger than 16 or the applicant’s signature if an adult. A non-refundable fee of $24 is charged for search of records. If the record is located, one certified or uncertified copy of the record will be provided at no additional cost. Additional copies requested at the same time at $15 each.
Uncertified birth certificates are available at the county register of deeds for people born in that county only. Allow six to eight weeks for mail-in requests.
Birth certificates in S.C.
Certified full copies of S.C. birth certificates may be obtained by writing or going to the Office of Vital Records, S.C. Department of Health and Environmental Control, 2600 Bull St., Columbia, SC 29201; 803-898-3630 or www.scdhec.gov.
Requests must include the child’s full name, full date of birth, the mother’s full maiden name, father’s name, the birth county and the signature of both parents if the person is younger than 16 or the applicant’s signature if an adult.
Certified, wallet-size birth certificates may be obtained for Horry County births by contacting the Office of Vital Records, 1931 Industrial Park Road, Conway, 915-8802.
Wallet-size certificates may be obtained for Georgetown County by writing or visiting the Office of Vital Records, S.C. DHEC, 531 Lafayette Circle, Georgetown, SC 29440. 546-0174.
Requests must include the child’s full name, the mother’s full maiden name, the father’s name, the birth county and signature of parent or applicant if older than 16.
Birth certificate cards for births outside the county may be obtained only by visiting the office. Wallet-size certificates are not acceptable for obtaining a passport for children younger than 18. The cost is $12 for the first copy and $3 for additional copies at the same time.
